First of all, it says on the cover, "500 USMLE-style questions." 80% or more of these questions are NOT in USMLE Step I format at all whatsoever. Some are extremely long (several paragraphs), some are 1-step reasoning (factoid memorization-recall), and many are testing non-pharm concepts or are just simply too easy.
